Revival heads back to Royal Stoke University Hospital to help patients return home safely

We’re thrilled to share that we’re once again providing support to enable people to return home from hospital safely.

Our home support provider, Revival, has joined North Staffordshire Integrated Discharge Hub (IDH), to help support people home from the Royal Stoke University Hospital.

Revival is no stranger to supporting the local NHS, with colleagues previously based at the hospital until last year through their Hospital Discharge service.

Now, in a new 12-month service, funded by the Better Care Fund, Revival will resume this support, helping patients and help those with low-level support needs to return safely home from hospital and prevent readmissions.

The Revival team will also play a vital role in helping the development of opportunities for partners in the Voluntary Community and Social Enterprise (VCSE) sector to ensure that local people have access to a wider directory of bespoke support to help people recover and settle back into their homes and communities.

The new service has created three new roles and a new Revival Discharge Team who will complete assessments and put support plans in place to help local people overcome any hurdles that prevent them from living independently.

Honeycomb Group Chief Executive, Julie Guildford Smith, said: “It’s fantastic news to see Revival head back to the Royal Stoke University Hospital.

“The team have supported the hospital for several years and have a expert knowledge of the hospital discharge process, and how to make it as smooth as possible for both patients and healthcare professionals.

“This includes removing any barriers to discharge, such as making sure a patient’s home is suitable to go back to and they have the support in place to allow them to recover well. Not only does this free up hospital beds and relieve pressure from hardworking hospital staff, it also prevents the chance of readmission, ending what can be a difficult cycle.

“This all relates back to Revival’s important mission of ensuring people can live at home independently for as long as possible and I couldn’t be prouder of what the team have achieved so far.”
